Here you will find VTEMS forms for Continuing Education, EMT Exam Application and Extension paperwork.  All forms are in PDF format.

Continuing Education Form This form is used to keep track of your CE needed for recertification
EMT Exam Application This form needs to be submitted NO LATER then 2 weeks before you would like to test.  this is for Recertifications only
Extension Form This form is used when and extension of your EMT card is needed
Communications Failure Form This form is used when attempts to contact SVMC with your pre-hospital report do not work
