Exam application - This form is for students to fill out to request spot for recertification test. Extension request - Form used if extension needed. State Protocols - Go to Policies and Protocols page. Continuing Education Form - To be submitted when at testing site. District #12 Competency Sheet - Competency sheet required to be submitted by all providers prior to testing. EMT Reciprocity - Form to be submitted to VT EMS from out of state EMT requesting Vermont card.
