     District #12 has started a new program called the Life-Saving Team Commendation.  Years ago, we used to recognize squads who had CPR saves, but decided to take this to the next level and recognize squads/teams that make a difference in the outcome of a patient.  The return of this program came after Deerfield Valley Rescue had a patient encounter that alot of people felt was in need of recognition (read below if interested).  This program is open to any EMS agency(s) or EMS provider(s) within the SVHC coverage area who was involved in the saving of a life based upon the following criteria:

     - Patient(s) was in imminent danger of death

     - ALS and/or BLS interventions, extrication or treatment was rendered

     - Interventions and Treatment was consisted with approved protocols

     - Patient was transported to a hospital and subsequently discharged from home to the hospital.

 

If you would like to nominate a provider or crew/squad, submit a completed application found below

     Life Saving Team Commendation form - This document is in Word format and this application may be filled out online and submitted

     Life Saving Team Commendation form - This form may be printed and submitted to the EMS Coordinator
